Ebola Attestation Requirement for The U.S.

(November 2026)

The U.S. Centers for Disease Control and Prevention (CDC) has issued a new order for international travelers entering the U.S. Effective November 3rd, 2026, at 11:59 P.M. EST, all travelers boarding an international flight into the U.S. must sign an attestation that they have not been in the Democratic Republic of Congo in the 21 days before boarding the flight to the U.S.

The Aircraft Operator must confirm that everyone onboard the aircraft has completed an attestation, and they must retain copies of the completed attestations for at least 42 days after the flight’s arrival into the U.S. The Aircraft Operator must provide the attestation to the U.S. Government upon request.

This requirement applies to both crew and passengers on all flights, including commercial, general aviation, and private. Anyone who fails to comply with this requirement may be subject to criminal penalties.

The CDC provides the Ebola Passenger Disclosure and Attestation Form that can be used by Aircraft Operators. Each traveler must sign their own attestation, except minors and persons physically unable to sign may have a parent, guardian, or authorized representative sign on their behalf.

Please do not hesitate to contact Flight Pro International with any questions you may have to clarify this new international travel requirement.
